Good incident management is not just about completing a form. Managers need enough information to understand what happened, how the child experienced it, whether plans remain right, what staff learned and whether anything needs to change.

Clear, objective recording that explains what happened, what staff did, why they did it and what the child experienced.

Notifications, chronologies, child and staff debriefs, restorative work and communication with relevant professionals.

Management review

Risk and plan reviews, patterns, proportionality, staff practice, learning and actions that are actually followed through.
