The Incident Is Over — Is the Work Actually Finished?

Once everyone is safe, the next phase begins. The quality of follow-up often tells you more about a home’s learning culture than the incident form itself.

Questions worth taking back to the home

  • What does the child say happened?
  • What did staff notice before escalation?
  • Do plans or risk assessments still reflect what is now known?
  • What needs restoring in relationships?
  • Is there a pattern the single incident cannot show on its own?

Do not confuse fewer incidents with better care automatically. Look at severity, context, lived experience and what changed because of learning.

Use this as a reflective prompt alongside the child’s plans, your organisation’s procedures, professional advice and any current regulatory or statutory guidance that applies.
